Try a pair of vice grips first. I pulled the exhaust manifold on one car to replace the rear stud only to find it came out easy. On the ohter hand all the other bolts broke. AS Bill said weld a nut on the stud. You can put a large washer behind the nut to avoid getting any splatter on the head. If you take apart an old hard drive from a PC you can get some nice super stong magnets. I use 'em to hold the nut in place.

My method is a reverse drill bit and heat, if the bolt is broken flush. If there is a "stud" heat, and vice grips have always worked. Of course the only time I have had to do this is with my engine out or on with the head off.
